Parental Consent Forms

Both notarized forms are needed when the minor(under 18 years of
age) is riding at FLWB Karting for the first time and their parent or
legal guardian is not present.  Once this form is validated, it is good
for one year and no additional consent form is needed.

If a parent or legal guardian is present, then we provide a
different form that does not need notarized.  Once this form
is validated, it is good for one year and no additional consent form is
needed.

*Effective Immediately: Only member minor consents will be kept on file. Consents filed prior to June 1, 2011 will be held until September 1, 2011 then discarded.

Waiver of Liability & Assumption of Risk

*Forms must be printed in COLOR and notarized by a
notary public to be valid.  (Don’t have a color printer?  Call us at
(812) 936-5390 and we will be glad to mail you the consent
forms.)

Forms are kept on file and valid for one year.  Once the consent forms
are presented, the minor can enjoy karting for the next year without
being accompanied by their parent.
